What to expect...

Whether you opt for the Telehealth session or schedule a Face-to-Face session, the goal is to assure your comfort before we proceed.

 

All clients are asked to fill out a Client Information Form (CIF) which will be kept on file and accessible through the Patient Portal. Your areas of concern are captured there, or feel free to forward a direct message through the Patient Portal.

At least 48 hours prior to your appointment, a Fields Bright associate will verify your CIF, forward a login invitation to set up your Patient Portal profile, verify your method of payment (proof or insurance will be required for filing claims), and confirm your appointment time. A *text-based/SMS appointment reminder will be forwarded to your phone number on file.
